You are free to search and find a senior community or retirement home by yourself or you can complete a senior application and allow us to find it for you. If you want us to provide you with the best matching facilities for yourself or your loved one then the APPLICATION FORM is the most important tool in achieving your goal. Complete the application to the best of your knowledge without withholding any information. We are here to help you in getting the best care that you need.

 

Using my administrator knowledge and years of experience, I designed the application form to protect your identity but, in the same time, to provide the facilities with every piece of information needed for a proper placement.

 

You will not be asked for sensitive information as social security number, date of birth, complete address, bank account information, Medicare or any health insurance ID, driver license number or any information that could lead to identity theft. Do not write any of these in the comment part of your application, nor the rehabilitation center or nursing home name in the event that you are not home. Certain medical information will be needed as well as the amount you are willing to spend for your care per month (but not the source of it).

 

Your application will be made visible to all senior communities and retirement homes in your requested area, as they have to be informed of your needs (medical, mental and physical). Some senior living facilities don’t accept certain medical conditions. You will be contacted only by the facilities that have bed availabillity, their fees are within your budget and they can accommodate and care for you and/or your loved one's needs.

 

IF  YOU  DON’T  AGREE  WITH US DISCLOSING  ALL, OR PART, OF THE  INFORMATION  IN YOUR  APPLICATION, DON’T  SUBMIT IT. WE  WILL  NOT BE  ABLE  TO  ASSISST YOU. 

BY  COMPLETING  THE  APPLICATION, YOU GIVE US  PERMISSION  TO SHARE THE INFORMATION WITH  ALL THE FACILITIES IN YOUR  CHOSEN CATEGORY AND LOCATION. NO OTHER MEMBER(S) OF THIS WEBSITE  NOR THE GENERAL  PUBLIC  WILL BE ABLE TO VIEW YOUR  APPLICATION.
